Athletic Physical Forms

  • This Parental Consent Form is required for all students participating in school sports. By completing this form, parents/guardians grant permission for their child to take part in athletic practices, games, and related activities. This form also acknowledges awareness of the risks involved in sports participation and provides emergency contact and medical authorization information as needed. Please complete and return this form (click here) to the school office prior to the start of the season.

  • This Physician’s Consent Form is required for student participation in athletics and other school activities as applicable. It ensures that each student has been medically cleared to safely participate in physical activities. Please have a licensed physician complete and sign this form (click here), confirming that the student is in adequate health and noting any relevant medical conditions or restrictions.

Medical Forms

  • This Physical Form is required for student athletes to ensure they are medically cleared to participate in school sports. It must be completed and signed by a licensed physician, confirming that the student is in good health and fit for athletic activity. Any medical conditions, restrictions, or recommendations should be clearly noted. Click here for the form.

  • This Dental Form is required to ensure students are maintaining proper dental health as part of their overall well-being. It must be completed and signed by a licensed dental professional, confirming that the student has received a recent dental exam and noting any concerns or recommended treatment.   • If your child qualifies for an immunization exemption, you may download the appropriate exemption form here. Please complete and return the signed form to the school office before your student's first day of attendance or as soon as possible.

Transportation Forms

  • For your convenience, bus transportation to and from school is available through several local public school districts.

    To request transportation, simply select the form for your school district below, complete the required information, and submit it to our office. We will review your request and forward it to the appropriate school district so they can include your student on their transportation route.

  • Students who possess a valid driver's license may be permitted to drive themselves—and, with parental permission, their siblings—to and from school.

    If you would like your student to be approved as a student driver, please complete the Student Driver Permission Form and return it to the school office either in person or by email.
