Athletic Physical Forms

This Parental Consent Form is required for all students participating in school sports. By completing this form, parents/guardians grant permission for their child to take part in athletic practices, games, and related activities. This form also acknowledges awareness of the risks involved in sports participation and provides emergency contact and medical authorization information as needed. Please complete and return this form (click here) to the school office prior to the start of the season.
